Speech-language therapy coverage and services in California

Medi-Cal pays for speech therapy for children under 21 when a physician or dentist writes the referral, and that referral is where most California families start. Private individual and small-group plans must cover habilitative speech-language services. Before age 3, Early Start routes your child through a Regional Center or school district. From age 3, the school district evaluates for an IEP. Because the referral window runs out after six months on Medi-Cal, ask the doctor for a fresh one before it lapses.

Medi-Cal speech therapy and the physician referral

Medi-Cal covers medically necessary speech therapy only on the written referral of a physician or dentist, delivered by a qualified Medi-Cal provider. The referral lists the diagnosis, requested services, frequency and goals, and it is limited to six months. For members under 21, the Medi-Cal manual says additional services may be available under EPSDT when medically necessary, and services that maintain a child's condition can qualify. The manual sets a general outpatient limit of two services per month, so if your child needs more, ask the provider to request it under EPSDT.

  • Ask your pediatrician or dentist for the written referral before you book an evaluation.
  • Track the six-month referral date and request the next one early.
  • Managed care members should ask their plan which speech providers are in network.

Habilitative coverage under Health and Safety Code 1367.005

Individual and small-group health plan contracts issued or renewed since 2017 must cover habilitative services. The statute defines them to include speech-language pathology and gives the example of a child who is not talking at the expected age. Habilitative services follow the same terms as rehabilitative services, and the two sets of limits cannot be combined. The benchmark plan summary CMS publishes lists speech therapy as covered with no visit limit stated, so ask your plan for its current limit, prior authorization rules and in-network list. Self-funded employer plans follow federal ERISA rules, not this statute.

Early Start speech and language services

Early Start serves infants and toddlers from birth to 36 months who have at least a 25 percent developmental delay in an area such as expressive or receptive language, or an established risk condition. Speech and language services are on its list of available services. Anyone can refer: call your Regional Center, your school district or the BabyLine at 800-515-2229. The Regional Center or district has 45 days to assign a service coordinator and complete evaluations. Evaluation, assessment and service coordination cost nothing, and insurance is used for medically necessary therapy such as speech.

School speech services under California Education Code

From age 3, children get special education through the local school district. A child qualifies as having a language or speech disorder when difficulty understanding or using language affects educational performance and a speech-language pathologist finds a speech sound, voice, fluency or language disorder, or a hearing-related disorder. Language and speech development and remediation is a designated instruction and service. After you request an evaluation, the district gives you a proposed assessment plan within 15 days. The IEP must follow within 60 days of your written consent.

Who licenses speech-language pathologists in California

State law bars anyone from practicing speech-language pathology or calling themselves an SLP without a license from the Speech-Language Pathology and Audiology and Hearing Aid Dispensers Board. A speech-language pathology assistant must be approved by the board and work under an SLP's supervision. School-employed SLPs can hold a Commission on Teacher Credentialing credential instead of a board license while working for a public school. California does not appear on the ASLP Interstate Compact map as a member state, so an SLP from another state still needs a California license.

Where California families find speech therapy

Speech-language providers on our directory cluster in San Diego, the Bay Area, Los Angeles and Fresno, with others across Orange County, the Sacramento area and Silicon Valley. For children under 3, your address decides the Regional Center, and in Los Angeles County the lines follow health districts rather than county borders. Check the DDS listing before you call.

Where to start in California

  1. Ask your pediatrician for a written speech therapy referral and note the six-month expiration date for Medi-Cal.
  2. Call your health plan and ask for its habilitative speech visit limit, prior authorization steps and in-network speech-language pathologists.
  3. For a child under 3, call the Early Start BabyLine at 800-515-2229 or your Regional Center and request an evaluation.
  4. For a child 3 or older, send your school district a dated written request for a speech and language evaluation.

California speech therapy questions

Do I need a doctor's referral for speech therapy through Medi-Cal in California?

Yes. Medi-Cal pays for speech therapy only on the written referral of a physician or dentist. The referral should name the diagnosis, the services requested, how often, and the goals, and it is limited to six months. Ask your pediatrician for it before you book an evaluation, and request the next one before the six months end so therapy does not pause. Members under 21 can receive additional services through EPSDT when medically necessary.

Does California require insurance to cover speech therapy for a child who is not talking yet?

For individual and small-group plans, yes. Health and Safety Code 1367.005 requires habilitative services and defines them to include speech-language pathology, with a child who is not talking at the expected age as an example. Plans can still use authorization and set a visit limit, but they cannot combine habilitative and rehabilitative limits. Large employer plans that are self-funded follow ERISA instead, so ask HR how your plan is funded.

Does Early Start charge for speech therapy in California?

Evaluation, assessment and service coordination are free. DDS says public or private insurance is used for medically necessary therapy, including speech, and the Regional Center or school district purchases or provides services that insurance does not cover. Start with the BabyLine at 800-515-2229 or your Regional Center. The 45-day clock runs from referral to a service coordinator and completed evaluations.

How long does my California school district have to evaluate my child for speech services?

The district must give you a proposed assessment plan within 15 days of your referral, not counting school breaks longer than five days. You then have at least 15 days to consent. The IEP must be developed within 60 days of the date the district receives your written consent, unless you agree in writing to an extension. Put your request in writing and keep a dated copy.

Can a speech-language pathology assistant provide my child's school speech therapy in California?

Yes, with limits. Education Code 56363 says language and speech development and remediation may be provided by a speech-language pathology assistant as defined in Business and Professions Code 2530.2. The assistant must be board-approved and work under a supervising SLP who answers for the extent, kind and quality of the services. Ask your IEP team who supervises your child's assistant and how often they meet.

Which California Regional Center handles Early Start if I live in Fresno?

Central Valley Regional Center serves Fresno, Kings, Madera, Mariposa, Merced and Tulare counties. Its Early Start intake for Fresno is 559-276-4300. In Los Angeles County the lines follow health districts, so use the DDS listing to match your address to a center. The BabyLine at 800-515-2229 can also point you to the right office.
