This Consumer Health Data Privacy Policy supplements SPCN's general Privacy Policy. It describes how Special Needs Care Network ("SPCN," "we," "our," or "us") handles information that may identify or allow an inference about a person's past, present, or future physical or mental health status or their attempt to obtain a health-related service. It is intended to provide the disclosures required by Washington's My Health My Data Act and comparable state laws when they apply.
Depending on the request, we may collect:
We do not request diagnoses, medical records, treatment histories, clinical notes, insurance member IDs, policy numbers, or images of insurance cards in our matching form.
We collect this information directly from the adult who completes a matching or inquiry form, from later communications that person chooses to send to SPCN, and from the person's interactions with SPCN's matching service. We may also receive limited response or routing status information from a provider that received an authorized request.

Before a matching request is submitted, SPCN asks the adult requester to authorize collection and use of the request details for matching. A separate authorization allows SPCN to share the request with relevant providers.
SPCN, rather than the requester, selects provider recipients through Lead Routing. The selection may consider service fit, geography, age information, service-delivery preferences, and eligible paid placement. Paid placement does not guarantee delivery and is not an endorsement.
After routing, SPCN emails the requester the names of the provider or providers that received the request. A provider may respond only about the requested services and using the contact method selected by the requester.
The requester may withdraw consent for future collection or sharing by emailing admin@specialneedsusa.com with the subject line "Health Data Request." Withdrawal does not invalidate processing completed before the request was received. SPCN will process deletion or downstream notice obligations required by applicable law.
When authorized or necessary to provide the requested service, we may share:
Provider recipients may include special-needs schools, therapy centers, individual therapy practices, advocacy organizations, attorneys, or other family-support organizations corresponding to the service categories selected in the request. We do not share consumer health data with advertisers, data brokers, or third parties for marketing unrelated to the request.
SPCN does not sell consumer health data. Providers may pay SPCN for advertising placement, but they do not purchase a family's information, and SPCN does not charge per inquiry, referral, or enrollment.

Email admin@specialneedsusa.com with the subject line "Health Data Request." We may take reasonable steps to verify the request. If we deny a request, you may appeal by replying with the subject line "Health Data Appeal." We will not discriminate against you for exercising a privacy right.
We retain request and consent records only for as long as reasonably necessary to provide the requested service, document routing and consent, resolve disputes, protect the service, and meet legal obligations. We use reasonable administrative, technical, and organizational safeguards, but no online system is completely secure.
SPCN's forms are intended for adults. The person submitting information about a child must be at least 18 years old and authorized to make the request. SPCN does not ask a child to submit contact information directly.
